University of the Ozarks Athletic Training

The athletic training facility at Ozarks is equipped with the latest technology to treat athletic injuries and enhance the athletes ability to rehabilitate quickly and fully. The athletic training staff oversees 10 athletic programs and approximately 160 athletes at Ozarks. Head Athletic Trainer Chad Floyd also provides a multitude of services in the athletic department such as organizing pre-participation physicals, physician referrals and post-surgical rehabilitation. He conducts a student athletic trainer workshop for prospective athletic training students during the summer.

Student-Athlete Physical Forms

All forms should be filled out completely and accurately. Please return completed forms to Chad Floyd upon arrival to campus. No student-athlete may participate in athletics until all paperwork has been completed and submitted.

The athletic training department is led by head athletic trainer Chad Floyd, who is in his seventh year as full-time athletic trainer. Floyd's vast experience includes working with Major League Baseball's Minnesota Twins and with two of the top collegiate baseball programs in Arkansas.

Floyd, a native of Ozark, Ark., began his athletic training career with the University of Arkansas-Fort Smith where he covered the nationally renowned Lion baseball program. Floyd then went to Arkansas Tech University where he covered baseball, football and basketball. After three years at Arkansas Tech, Floyd worked with the Twins minor league system at several locations including Fort Myers, Florida, Fort Wayne, Indiana and Davenport, Iowa. Floyd's duties with the Twins included daily treatments, rehabilitation and game coverage. He also helped coordinate a daily in-season conditioning program. Following his stint with the Twins, Floyd worked two years with Cooper Clinic Sports Medicine where he oversaw numerous area high schools and served as a liaison to the clinic for injury referral.

Floyd holds state, district and national training certifications and has an Arkansas teaching certificate in Health and Physical Education. He is a member of the National Athletic Trainers Association, Southwest Athletic Trainers Association and Arkansas Athletic Trainers Association. He currently serves on the Arkansas Athletic Trainers Association scholarship committee. Floyd is a 1995 cum laude graduate of Arkansas Tech University where he earned a Bachelor's degree in Health and Physical Education. He currently teaches Care and Prevention of Athletic Injuries at Ozarks.
